You can prepare almost everything in India, but whether you may work in Austin depends first on your immigration work authorization, and your MBBS alone does not authorize Texas physician practice.

I am an Indian MBBS graduate moving to Austin, Texas after marriage. What should I prepare while still in India regarding finances, driving, and healthcare, and what are my options for working in healthcare while pursuing the USMLE/ECFMG pathway?

Summary

You can do much of the slow work—document collection, ECFMG setup, financial records, medication preparation, and driving paperwork—before leaving India. You also have legitimate ways to stay in healthcare, but paid work must wait until your immigration document authorizes it, and physician work requires a Texas permit or license.

Your options fork first by immigration work permission, then by whether you seek bridge work, residency, or Texas's experienced-foreign-physician route.

Standard IMG-to-residency routemain route

Complete through the correct 2027 , pass Step 1 and Step 2 CK, obtain a residency position, and train under a Texas . TMB restricts that permit to supervised practice within the training or fellowship program; full Texas licensure generally requires two years of qualifying progressive training.

Healthcare bridge job while studyingif authorized

If your status already gives or USCIS has approved your , you may pursue employer-defined non-physician work such as clinic administration, scheduling, medical scribing, or research coordination. For hands-on regulated jobs such as nurse aide or pharmacy technician trainee, obtain that occupation's separate Texas credential first; never diagnose, prescribe, or hold yourself out as a Texas physician.

Texas foreign-physician provisional routeif experienced

This is viable only if you hold the required foreign background: ECFMG certification, Steps 1 and 2, same-specialty postgraduate training, at least six months of practice in the prior two years, and a qualifying shortage-area job offer. The current TMB application fee is $602 plus $21 and $7 surcharges; successful initial and second provisional practice within six years is required before seeking full licensure.

Work directly as a doctor on the MBBS alonenot enough

Your Indian MBBS by itself does not authorize physician practice in Texas. Until you hold a Texas-authorized training permit, provisional license, or full license, limit employment to duties that do not amount to physician practice.

Immigration and professional licensing are separate—you need both whenever the job itself is licensed.

Watch out for

Marriage does not automatically mean work permissionYour immigration document controls. A Green Card permits work; an E-1S/E-2S/E-3S/L-2S spouse is work-authorized through status; an H-4 spouse needs to qualify for and receive an ; a pending I-485 alone is not enough; and F-2/M-2 or B-1/B-2 status does not permit employment (USCIS Green Card page; USCIS Policy Manual Vol. 10, Part B, Ch. 2; USCIS Policy Manual Vol. 2, Part F, Ch. 9; State Department Visitor Visa page). Do not start even an unpaid role that provides productive services until your branch permits it.
An MBBS is not a Texas physician licenseYou cannot work as an independent physician merely because you hold an Indian MBBS. The usual route is , a U.S. residency under a Texas , and later full licensure; Texas says IMGs generally need two years of continuous progressive accredited or Board-approved training for a full license (TMB Full Texas Medical License Application).
The new Texas foreign-physician route is narrowThe provisional route is not a shortcut for every IMG. It requires ECFMG certification, Step 1 and Step 2, an employment offer, same-specialty residency or similar postgraduate training, at least six months of medical practice during the two years before applying, and practice in a qualifying shortage area; both provisional stages must be completed successfully before full licensure (TMB Foreign Physician Provisional License page).
The 2027 ECFMG clock is already runningFor the 2027 Match, the application, supporting documents, and OET results must reach ECFMG by January 31, 2027; ECFMG recommends taking OET Medicine no later than the last available December 2026 test date. Your Indian license does not waive Texas testsTexas allows a valid qualifying foreign license for only up to 90 days after moving. India is not among Texas's reciprocity countries, so plan for the knowledge and road tests; obtain an official translation before arrival if the license is not in English or Spanish (Texas DPS Moving to Texas).
Cash and foreign accounts have separate reporting rulesIf your family collectively carries or sends more than $10,000 across the U.S. border, report it to CBP on FinCEN Form 105. Use these branches: a Green Card or temporary I-551 evidence permits work; an unexpired E-1S/E-2S/E-3S/L-2S I-94 permits work incident to status; H-4 requires eligibility plus an approved Form I-765/EAD before work; a pending I-485 permits you to apply on Form I-765 but is not itself work permission; F-2/M-2 and B-1/B-2 do not permit employment. Keep copies of the passport identity page, visa, I-94, marriage certificate, and any EAD.

Collect your ECFMG and residency records in India

Confirm that your school and graduation year have an ECFMG Sponsor Note in the World Directory. Create MyIntealth and submit the diploma copy there—never mail the original—then make sure your school can answer ECFMG's primary-source verification and provide the final transcript. If you held unsupervised registration on or after January 1, 2022, use Pathway 1 and have the council send the good-standing/current-status letter to licensure@ecfmg.org by January 31, 2027; if uploaded by you, it must be issued within 90 days. If you never held unsupervised registration, Paths 3–5 apply only to qualifying recent graduates and schools; otherwise Pathway 6 uses six in-person Mini-CEX encounters.

Complete the 2027 certification milestones

Use https://www.ecfmg.org/certification-pathways/. Take OET Medicine no later than the final available December 2026 date. ECFMG must receive OET scores, the Pathway application, and all supporting documents by January 31, 2027. ECFMG Certification is required for IMGs to enter accredited U.S. graduate medical education and is an eligibility requirement for Step 3.

Before travel and throughout the move year

Prepare your money trail and U.S. banking packet

If your traveling family collectively carries or sends more than $10,000, report it to CBP; electronic FinCEN Form 105 filing is available at https://fincen105.cbp.dhs.gov/. Keep maximum-balance and income records for every Indian account. After you become a U.S. tax resident, U.S. taxation generally reaches worldwide income; an aggregate foreign-account balance over $10,000 can trigger FBAR, and a married couple filing jointly while living in the U.S. can trigger Form 8938 above $100,000 at year-end or $150,000 at any time. For banking, CFPB confirms an SSN is not universally required, although each institution sets its identity-document policy.

Before departure and immediately after arrival

Arrange health coverage and bring a medical packet

If still within 30 days of marriage, your spouse should request special enrollment in the employer plan now. If that route is unavailable, a lawfully present move from India to the U.S. can qualify for a Marketplace Special Enrollment Period at https://www.healthcare.gov/coverage-outside-open-enrollment/special-enrollment-period/. Bring no more than the FDA's stated 90-day foreign-national supply of medicine, in original containers bearing the doctor's instructions, plus copies of prescriptions and vaccine records.

Within 90 days after moving

Convert your driving status promptly

After establishing Texas residence, schedule DPS Driver License Services. India is not on the reciprocity list, so prepare for the vision, knowledge, and road tests. DPS requires two printed residency documents, and one normally must show at least 30 days in Texas. The Class C fee is $33 and the license lasts eight years for ages 18–84; age 85+ is $9 for two years. Applicants aged 18–24 may also need the six-hour adult driver-education course. Register any vehicle you own in Texas before applying.

Choose only work your status and credential permit

Before residency, target duties that do not involve independent physician practice: clinic administration, scheduling, medical scribing, research coordination, or other employer-defined support work. Nurse aide and pharmacy technician trainee are possible regulated alternatives only after their separate Texas requirements are met. Once matched, the residency program initiates the Texas PIT process; the permit is restricted to supervised practice within that program. Use the foreign-physician provisional application only if you meet every recent-practice, postgraduate-training, exam, job, and shortage-area condition.
