Summary

A bank’s information form and a 0% withholding rate do not, by themselves, mean you owed tax or needed to file. The notice identifies a problem with the return you already sent; you can explain it without inventing a taxpayer number, though the IRS has not published a guaranteed no-ID resolution for this form.
